    Complete your intake forms

    Arrive 10 minutes early

    For an in-person visit, arrive about 10 minutes early with your ID and insurance card. For telehealth, find a quiet, private spot and click your link at the appointment time.

What to bring to your first visit

A little preparation makes your first appointment smoother and frees up more time to focus on you. Bring these four things:

  • Photo ID

    A driver's license or other government-issued photo identification.

  • Insurance card

    Your current insurance card (front and back). Self-pay is welcome too — just let us know.

  • Medication list

    A list of current medications, supplements, and doses — or simply bring the bottles.

  • Prior records

    Recent lab or imaging results and the names of any specialists or your pharmacy, so we can coordinate.

Do I need a referral to become a new patient?
No. Primary care is your front door to the health system, so you can book directly as a new patient — no referral required. We'll refer you to a specialist only if and when you need one.
Can my first visit be a telehealth appointment?

What should I bring to my first appointment?
